We are very pleased that your student has chosen to participate in the Douglas High School Athletic Program. Douglas High School has earned respect around the state for the hard play of its athletes and the sportsmanship of its players, coaches, and fans.
New Sports Physical Forms and Medical History forms for the 23-24 School Year.
NIAA has updated the Sports Physical and Medical History forms as well as the process for filing these. Our old forms will no longer be accepted. Effective Immediately, all athletes must have a NEW physical, even if they had one in the last year. Make sure you take the New NIAA History Form, New NIAA Physical Form and Medical Eligibility Form with you to your appointment.
Doctor must complete and sign, this is the only form parents scan & upload to Aktivate when registering a student for a sport.

Sports Physicals - Additional Information
The Carson Tahoe Urgent Care near Reds 395 Restaurant in South Carson gives sports physicals from 8am-8pm, 7 days a week for $55, no insurance, walk-ins welcome. PLEASE NOTE: Students must bring all the new NIAA blank forms with you, they do not have any of the new forms.

- ALL WALMART CLINICS ARE CLOSED.
- CARSON VALLEY HEALTH (formerly Minden Family Medicine) on Lucerne across from closed Sonic Burger, performs sports physicals with an appointment. They are free on Thursdays to the first 10 students making an appointment, $25 any other day of the week, they don't take insurance. Call 775-782-1603 for an appointment and have the medical history form completed before you go, one parent only.

When your son or daughter participates in any sport at DHS, you automatically become a DHS All Sports Booster. Boosters is a great way to help support DHS athletes and also an awesome way to meet other parents/families that attend DHS.

PURPOSE
The DHS All Sports Boosters’ primary purpose is to coordinate fundraising efforts to support all of the DHS sports programs. The bulk of the money raised is through snack bar sales and DHS Event Passes but the money raised benefits all the sports in the club.

EVENTS PASSES
Our Booster members are also encouraged to buy and also help sell Events Passes. Anyone who purchases Event Passes saves money and all of the money collected goes directly to the sports programs through the All Sports Boosters club. MEETINGS
Booster’s meetings are the 1st Monday of the month (Unless it falls on a Holiday/District in Service day then the meeting will be the following week). The meetings are held in the commons at 6 pm. We welcome everyone to join the All Sports Boosters and help in any way you can. If you cannot volunteer in the snack bar but have other skills or connections to help our program we would be happy to hear from you. Athletes must be cleared (in green) on Aktivate in order to participate/try out.
Remember, all athletes need a new physical on the new NIAA forms.

We must all work together to support the efforts of our athletes. As spectators we represent our school just as members of the team do. Thank you for demonstrating your Tiger Pride by being a positive influence on our athletes, coaches, officials, and fans. We will do all we can to provide a positive experience for your athlete. To support this experience, we have found it essential to establish open lines of communication between athletes, coaches, parents, and school staff. This allows for quick resolution to questions and concerns before they become issues.
