Please complete the 2 page HISTORY form in ink and have the student's name at the top all forms
prior to checking in at the Free Sports Physical Night.

 

Sports Physical Forms and Medical History Forms

NIAA has updated the Sports Physical and Medical History forms as well as the process for filing these. Our old forms will no longer be accepted. Effective Immediately, all athletes must have a NEW physical, even if they had one in the last year.  Make sure you take the New NIAA History Form, New NIAA Physical Form and Medical Eligibility Form with you to your appointment.
