DHS Common Application Packet (CAP)

The DHS Common Application is often referred to as the DHS CAP. For most local scholarships you will be able to use the Douglas High School Common Application Packet.  Follow these steps when completing the common application:

  • Douglas High School Common Application Packet Form (Google Doc, PDF)

  • Personal Statement Autobiography - Guidelines PDF

  • At least three letters of recommendation (Request Form PDF)

  • An unofficial transcript  (email a request from your student email to dhsstudentrecords@dcsd.k12.nv.us )

  • Be sure to create a simple cover sheet for each scholarship which shows your name and the name of the scholarship you are applying for.

  • Use only ONE staple to secure your entire packet. If you have parts stapled together, such as letters of rec or autobiography, take out these extra staples before the final staple.

  • DO NOT (1) use a paperclip, a binder clip, or other such fastener; (2) use a folder, plastic sheet protector, or other covering for your packet.

  • Put your packet in the following order: cover, CAP form, autobiography, letters of rec, transcript.

Free Application for Student Aid

Many need-based scholarships require completion of the FASFA.

Many states and colleges set priority deadlines by which you must submit the FAFSA form to be considered for the aid programs they administer. There is also a federal deadline each academic year.

Scholarship help is available every Wednesday and Thursday during lunch in DHS room 302. Bring your resume and get help building your CAP or fill out an organization specific application for scholarships. Printing is available.

Three Levels of Senior scholarships

 

National scholarships are open to any high school senior graduating across the country.

 

Here are a few general resources to get you started as you continue your national search of scholarships by your career goals and post-secondary institutions.

Regional scholarships are open to students in the state of Nevada or select regions of Nevada. Many are connected to specific colleges or universities and often have early deadlines associated with them.

Local scholarships are open to Douglas High School or Douglas County graduates specifically. These are provided by local businesses and organizations to help our students who qualify based on the sponsoring organization's requirements.

Many of our local donors use the Douglas High School Common Application Packet. See this side-bar section for help assembling your CAP with an autobiography, three letters of recommendation, transcript, and general information form.

Local Scholarship Applications

Local scholarships may be turned in to DHS room 302 before school, at lunch, or after school. Please do not turn in applications during passing or class periods. 

  • Do not turn in applications in an envelope
  • Please use a cover page as directed for the DHS CAP when turning in an application
  • Students must sign in each application for each scholarship they apply for. This process takes a couple minutes, therefore, please plan to turn in applications before school, at lunch, or after school.
Scholarship awarDS

Scholarship award night will be held at LifePoint Church at 6pm on May 24, 2023. LifePoint is located 1095 Stephanie Way, Minden, NV 89423. Students receiving an award will be invited by letter delivered to 5th period by May 17.

Following announcement of your award, students are expected to write a thank you and provide their school/program information to the sponsoring organization.

Scholarship money will be mailed directly to the school you indicate on your information form. Please return your student information form directly to your donor or drop off in room 302 by June 2, 2023.

Criteria: cover sheet, info sheet, application description

1. ESSAY:  The essay should discuss your interest in your major area of study and areas of specialization and the occupation you propose to pursue after graduation.  What are your long term goals and how do you hope to achieve them?  The essay may also include any other information that is relevant to your career plans. The essay should be typewritten or computer-printed essay of no more than 500 words, double spaced. Put your name and major field of study at the top of the page.

 2.  COPY OF SACREMENT OF CONFIRMATION CERTIFICATE.

3. VOLUNTEER ACTIVIIES:  A list of volunteer activities, extracurricular activities during high school including any position held such as President, Secretary, etc.  WORK EXPERIENCE:  A list of employment during school and summer break.  HONORS AND SCHOLARSHIPS:  The list may or may not be academic related.

a.    Complete Application.

d.    At least two (2) Letters of Recommendation as evidence of your college achievement potential and past performance in school or in your community. Letters may be from teachers, administrators, coaches, business, or civic leaders.
e.    Include with the application an essay summarizing your future goals.

Sponsor: Brooks Family

Details: The Brooks Family Scholarship aims to help students that are motivated to build a successful
career and life for themselves, whether it be through a 4-year college program, a junior college or
a trade and technical school. This scholarship is not necessarily looking for the perfect student,
but for a motivated individual who is hard working and disciplined. This scholarship may be
used in or out of state.

Criteria: strong work ethic toward continuing education with strong life goals
preference given to: character (5), leadership (5), financial need (5), education goals (5), GPA (3), extracurricular activities (3), athletics (3), community service (2), specific field of study

Award: $1,000

A 3–6-minute audio or video recording answering the following questions:

  • What are your goals for the next year?
  • What skills do you have and how do you intend to apply them towards a career?
  • In the real world, it is not just academics that contribute to your success, what other skills do you possess that will help you succeed?

Eli Weigel and Nicole Snyder Memorial Scholarship

Criteria: .C.Meneley attendance at least 1 full year, financial need, GPA, community, extra-curricular, job experience, professional goals

Criteria: Resident and student of Douglas County, Nev.; Proof of acceptance into an accredited higher education institution; Minimum 3.0 grade point average

Criteria: The student must be enrolled in a program for a Bachelor’s degree.

• In high school, the student must have maintained a 2.5 or better GPA, with a 3.0 GPA during the senior year.

• The student must have demonstrated a concern for the betterment of high school and the community in one or more ways.

• The student should present at least THREE letters of recommendation in support of his/her potential success in post-high school education.

• Interested and qualified students should complete the scholarship application form available at the counseling office and on the Carson Valley Sertoma website, meeting the established criteria and deadline for submission.

• Finalist must be willing to be interviewed for the CARSON VALLEY SERTOMA MEMORIAL BOOK SCHOLARSHIP.

• If awarded the scholarship, the student must be willing to attend the Carson Valley Sertoma Scholarship Breakfast in May.

Award: A four-year renewable scholarship for textbooks. This scholarship will be paid in renewable increments each semester.

Criteria: The student must be enrolled in a program of a vocational training curriculum (auto tech, nursing, medical tech, culinary, skilled craftsmen, truck driver, welder, cosmetology, etc.).

• In high school, the student must have maintained passing grades.

• The student must show at least one activity demonstrating betterment of the high school and/or community.

• The student should present at least TWO letters of recommendation in support of his/her potential success in post-high school education.

• Complete this scholarship application form with all the information requested before the deadline.

• Finalists must be willing to be interviewed for the Carson Valley Sertoma Club Vocational Scholarship.

• If awarded the scholarship, the student must be willing to attend the Carson Valley Sertoma Scholarship Breakfast in May.

3. Live within Liberty Utilities’ service territory

4. Must be accepted to an accredited vocational trade school, plan to pursue an apprenticeship or be accepted to an accredited 4-year or 2-year university or college

5. Must plan on pursuing a vocational trade or academic degree that supports construction, arboriculture, engineering or renewable energy or any other academic pursuit that would fulfill other utility related fields such as business, accounting, legal, communications, renewables, IT, etc.

Criteria: Scholarship Rules

A. The contest is open to those seniors who will graduate at the end of his/her current senior year and plans on a career in Medical or Forestry programs. Also is a resident of Douglas, Carson, Lyon, Churchill, Landers, Eureka, or White Pine Counties.

B. Any senior who is a member in good standing or is the child of a parent that is in good standing of the Nevada Division of the National Pony Express Association is automatically eligible to participate regardless of County of residence.

Award: $1,000

Pop Warner Alumni Scholarship is a $1000 Scholarship awarded to one past Cheer and Football participant who is looking to further their education at a university or trade school.


Applicants must submit an essay on how Douglas Pop Warner impacted their lives.

Phyllis Robison was an educator in Douglas County for 26 years, and valued community service and giving back to the community above all else. She would tell her daughters that “in life it really didn’t matter how smart you were or how good looking, what truly mattered is that you have a good and giving heart.”

Jim Summers grew up in the Carson Valley. He graduated from Douglas High School in 1952 and was an All-State football and basketball player. In 1952, in a home game against Stewart, Jim scored 70 points in a single game which still stands as a Nevada single game scoring record. Jim passed away in September 2021. 2023 is the first year of the Jim Summers Memorial Scholarship and his family looks to honor his legacy by rewarding 2 student athletes.

Criteria: Letter in Football, Boys Basketball, or Girls Basketball;  Unweighted GPA 3.0 or better; Pursuing University, College, Community College or Trade School; Financial Need

The award winner will receive the scholarship based on academic success, participation in extracurricular activities and community service, and plan to attend a vocational training program.

Award: $1,000 (academic - bachelors program and vocational-2 year associate's degree program or 1-year tech training)

Criteria: At least a 3.3 or above GPA (unweighted); Graduating high school senior pursuing a degree relating to the field of agriculture.  We want to see their; interests and activities as well as financial needs. The student does not need to be involved with the cattle industry. We are looking for well-rounded individuals who will continue school in the United States.

Award: $5,000
